Insurance Information
Westside Primary Care Associates accepts most major health insurance plans.
Payment is expected at time of service. Payment may be made by cash, personal check, Visa, MasterCard or American Express.
We will submit all claims for the insurances with which we participate and will provide you with a copy of what was performed on that day to submit to your insurance company if we do not participate. The bill remains your responsibility.
This is a general list of plans accepted and should be used only as a guide. This list is subject to change. If you are uncertain as to whether your individual health insurance plan is accepted at Westside Primary Care please call your insurance company.

*Some insurance plan exclusions apply. Westside primary care providers do not participate with all health plan products. Please contact your insurance company to confirm if your health benefit plan includes receiving in-network services from Westside Primary Care Associates providers.
Referrals
Many insurance plans require referrals which need to be obtained prior to any visit with a specialist. Referrals will be mailed to the patient or can be picked up from the office. Except in a medical emergency, we will not fax referrals. Referrals cannot be back-dated, as this violates the agreement that has been made with your insurance company. We are not responsible if your specialist allows you to see them without the proper referral. Except in the case of an emergency, we will require 48 hours notice to process your referral. You may also request and receive referrals via the patient portal.
Referrals that are requested for pick-up will be ready in 48 hours, please be prepared to show identification.
